- Claims should be submitted within 90 days
- Ensure that you take along GTM’s official claim form
when visiting your Provider
- State your Policy number on all correspondence/claims etc.
Include the date the service was rendered and be sure to remind
the Provider to document the Diagnosis (es)
- The type of treatment received (surgery or otherwise) should
be clearly stated
- An itemized list of Prescription Drugs and Diagnostic procedures
should be clearly stated
- All claim forms should be signed by the Insured, dependant
and Provider and stamped by the Provider as well.
- All original receipts should accompany claim forms
- Once a client visits a Specialist the name of the referring
General Practitioner should be stated on the form. |
